A scathing report on pediatric forensic pathology in Ontario released today points the finger at disgraced pathologist Dr. Charles Smith and his direct supervisors at the time, Dr. James Young, the Chief Coroner and Dr James Cairns, the Deputy Chief Coroner. An inquiry was launched in April 2007 after errors were found in 20 of 45 autopsies performed over a ten year period by Smith. Justice Stephen Goudge led the probe, has released a four volume review and has made 169 recommendations.
